Prayer card is printed on cream cardstock paper with an illuminated manuscript style design printed on the front and back of the card. 

Front of the card at top has gilded, red, and blue leaf forms set within curling black lines resembling vines; within this decorative element is a circular black and white image of President Kennedy from the shoulders up. There is text printed in black below with the first letter of three of the words printed in color and gilded. The text is as follows:
"In Memory of 
John F. Kennedy
35th President of the United States
Born May 29, 1917
Inaugurated January 20, 1961
Died November 22, 1963

May his soul and the 
souls of all the faith- [sic]
ful departed, through the 
mercy of God, Rest in Peace.
... Amen".
A smaller version of the illuminated design at the top of the card is repeated in a narrow border at the bottom of the front of the card with a gilded cross in the center of the design.

In miniscule text printed in black at the lower left edge of the front of the card is the text "(c) 1962 J. O. Mooney CO. N.Y.C. M-2". The date appears to be a misprint, stating a copyright year of 1962.

The reverse of the card has "Let us pray" in gilded lettering followed by a prayer printed in black underlined text with various letters illuminated in blue, red, and gold. The same motif that surrounds the black and white image of President Kennedy on the front of the card is repeated in a smaller gilded version at the bottom of the reverse of the card underneath the prayer. The text of the prayer is as follows:
"O Gentlest Heart of Jesus,
ever present in the Blessed
Sacrament, ever consumed
with burning love for the
poor captive souls in
Purgatory, have mercy 
on the soul of Thy servant

Be not severe in Thy judg-
ment but let some drops 
of Thy precious Blood fall 
upon him, and do Thou O
merciful Saviour [sic] send Thy
Angels to conduct him to a place
of refreshment, light and peace.
... Amen
May the Angels lead thee into
Paradise.
May the Martyrs receive thee
at thy coming and take thee to
the Holy City.
May the choirs of the Angels 
receive thee and mayest thou have
rest everlasting.
... Amen".

In the lower left corner, the publisher's name and copyright date are printed; the date appears to be a misprint, stating a copyright year of 1962.
