The image was taken by a photographer from The Dallas Morning News.\r\n\r\nThe details in the document are not completely clear, but it can be seen that Lee Harvey Oswald signed the document titled, \"International Certificate Of Vaccination Or Revaccination Against Smallpox.\"  On a chart in the center of the document, it is shown that on June 8, 1963 a \"Dr. A.J. Hideel\" of New Orleans administered the smallpox vaccine to Lee Harvey Oswald; a stamp of approval was applied to the chart.\r\n\r\n\"KODAK SAFETY--FILM\" is printed at the top of the negative.
