Jones at Parkland Memorial Hospital in Dallas, Texas dated Saturday, November 23, 1963. This is a summary statement that describes what Dr. Perry did and saw in the Parkland Hospital emergency room during the treatment of President John F. Kennedy on November 22, 1963. Dr. Jones was the third physician to treat the president and administered fluid to the left arm to help stabilize him. The statement is handwritten in blue ink on the front of an Admission Note form. There are also 2 copies of this document.
The statement reads:
"Summary of Treatment of the President / 11-23-63
After receiving a stat call from the hospital operator that the President had been shot and was en route to the emergency room I immediately notified Dr. M. T. Jenkins and Miss Audrey Bell who were nearby.
Myself and Dr. M. O. Perry rushed to the emergency room and found the President to be dying. Previously described severe skull and brain injury was noted as well as a small hole in the anterior midline of neck thought to be a bullet entrance wound.
While Dr. Perry started the tracheotomy I performed a cut down on the left cephalic vein and inserted a large polyethylene catheter to which was attached ringers lactate solution.
It was then noted that air was bubbling through the neck wound and I inserted a left anterior chest tube in the 2nd interspace(?) in the MCL using a trochar(?) and heavy rubber tubing. This was connected to closed chest drainage immediately. In a similar fashion Dr. Paul Peters, Dr. Charles Baxter, and myself inserted a right anterior chest tube just to the right of the mid [?] line and attached to closed underwater drainage.
Subsequently closed cardiac massage was began and the patient expired.
